Information for Professionals
Pathfinder is a 3-year homelessness prevention programme for young people across Greater Manchester...
The Pathfinder programme has been designed for young people who are either homeless or at risk of homelessness. Through coaching, it enables these young people to overcome any challenges that are impacting the stability of their accommodation circumstances.
Our priority is to work with 18-25 year olds, but we are able to support people up to the age of 35. Over the duration of the programme, we aim to support 1,250 young people across all boroughs of Greater Manchester through our specialist local delivery providers.

We take a person-centred approach to our work with young people, tailoring support to meet their individual needs. The support focuses on four goals:
-
Long term improvement of housing situation
-
Increased financial stability
-
Improving physical and mental health
-
Increasing skills and access to opportunities

To achieve these goals, we commit to:

Build trusting and respectful relationship with every young person

Act as a lead professional, coordinating other support agencies as needed

Offering every young person support tailored to allow them to achieve their goals

Involving the families of young people through support and mediation techniques where appropriate

Seeking out and securing meaningful opportunities for every young person

Use a personalised budget to enable young people to access accommodation/ education/ training/employment and meaningful activity
